The data from these forms will only be used for the administration of services provided by the Library and Knowledge Services. By completing any of these forms you agree to the data being held in accordance with the Data Protection Act (DPA) 2018 / UK General Data Protection Regulation (GDPR) for use in any relevant local library systems and/or external alerting systems. 

We may store personal information supplied via these forms to enable us to identify you, collate statistics, track progress in satisfying requests and to remedy problems.

We will not share person identifiable information with third parties outside your employing organisation unless a Data Processing Agreement is in place. The Library and Knowledge Services has a Data Processing Agreement in place with Health Education England (the Data Controller) for the shared Koha Library Management System. This is fully compliant with GDPR.
